What to Expect: Days 1 and 2
Day 1
Patient Forms
As you may expect, Dr. Lee will ask you to fill out some forms to gather much needed information about you and your condition. All forms will be provided when you arrive, but you can choose to fill them out prior to your visit. Just click New Patient History under the Online Forms, print, and complete. Don't forget to bring them with you to your first visit!
________________________________________Consultation
Dr. Shawn R. Lee will review your forms and have a consultation with you. Please feel free to discuss your health care goals, current symptoms, concerns, any injuries or accidents, health history, and everything the doctor may need to know when determining your diagnosis. Healthcare can be scary for some people, but we want you to know that you have come to the right place. It's Dr. Lee’s mission to find the cause of your condition and offer you solutions to get you back to where you'd like to be in your health.

Examination
To find the cause of your condition, we will need to gather as much information as possible. A complete examination will be performed. The doctor will be using standard neurologic, orthopedic, postural, and physical examination to pinpoint the exact nature and cause of your condition. The results of these tests will help Dr. Lee customize a treatment plan just for you.
________________________________________X-Ray Studies
To fully evaluate your condition X-rays may be needed and for your convenience we provide X-rays onsite! Since nobody has x-ray vision it is the only way we can see what is happening inside your body. If you have recent films or MRI’s please bring them with you we will be happy to review them.
